Let us know a little about yourself to help us get you started with our Early Childhood Education Program. Please Note: Completing this form does not guarantee enrollment. Our intake staff will contact you to set up an appointment to cover eligibility requirements, availability, etc.

  • Family Information

    We'll cover more specifics during your intake appointment, but your responses here can help us get started with the process. Our Early Childhood Education program serves kids ages 2-5; please provide names and birthdays for your children who meet these age requirements. Children between 18 and 24 months can be added to our wait list, so please include them here if you wish to enroll them when they are eligible. If you're interested in our Home Visiting programs for expectant families, newborns, and toddlers, please make a note in the "Additional comments" area.
